South County Home Health’s new Diabetes Management Team provides comprehensive strategies for those patients who are living with the disease. Each member of the team is passionate about helping patients manage this complex diagnosis so they can enjoy a higher quality of life.
The team members (pictured left to right) include a nurse, Samantha Fuimarello, BSN; pharmacist Julia Manning, R.Ph., CDOE; dietitian/nutritionist Sarah Marotto; and occupational therapist Sue Hunt.
Adding an occupational therapist with advanced education in diabetes care brings a set of unique skills to the team. Vision loss, dexterity and cognitive issues may impede a patient’s ability to take medications at the right time or administer insulin. Sue can assist with these functional issues, while bringing a thorough knowledge of diabetes to each case.
In addition to seeing patients, South County Home Health’s Diabetes Management Team provides guidance to clinical staff. The response from patients far has been overwhelmingly positive as we continue working with patients and providers to facilitate the best possible outcomes for our patients.
