Coordinated care makes for better care. The South County Medical Group is a comprehensive network of expert specialists and primary care healthcare providers created to provide a continuum of care for patients. The group includes primary care physicians and specialists.
Together, these practices give patients easy access to specialists as needed and, by working together as a cohesive team, can better manage each individual’s healthcare needs.
One way in which this is done is the sharing of the same information systems platform for patients’ Electronic Health Records (EHR). Access to up-to-date and complete health records is vitally important in the diagnosis and treatment of injury, illness, or chronic disease. When a person sees several different physicians for a multitude of health issues, it’s particularly important that all physicians involved be aware of all treatments being delivered. Since most physicians do not share a database platform, this feature of the South County Medical Group members is an important point of distinction.
